2010-2011 - Player Registration Checklist

Step 1: Complete ‘try-out acceptance’ online using the link in the email received from the club after tryouts.

Step 2: Come to your scheduled registration night to complete your registration with both the club and the league. If you are unable to attend any of the registration sessions below please contact Cindy Mochel at cindymochel.hawkssoccerclub@comcast.net.

What to bring:

• Your player’s IWSL (girls) or NISL (boys) player registration form. This can be pre-populated and printed from our website during your tryout acceptance.
• 2 copies of the complete medical waiver form (per player). This can be pre-populated and printed from our website during your tryout acceptance. Girls will need the IYSA waiver. Boys will need the US Club AND the IYSA waiver.
• 2 copies of your family’s medical insurance card (per player)
• A copy of your birth certificate (if new to the Hawks)
• A check to pay your deposit and uniform order (if not paid on the website)
• Uniform sizing will be available at Registration. You will be sent a link to order your uniform online.
• A separate check for Hawks Spiritwear, if you should choose to order some.

Registration will be held at Union Church of Hinsdale 137 S. Garfield Ave, Hinsdale, IL
